How to Give Feedback or Make a Complaint

Speaking up is your right.

You should feel safe to raise concerns about your NDIS supports.

You can provide feedback directly to your provider if you feel comfortable doing so. A registered provider must have a complaints management and resolution system and provide participants with information about how to complain, external complaint pathways and access to advocates.

At Finding Path

You can speak with our team, contact our Clinical Care Manager, email complaints@findingpath.com.au, or use our secure Feedback & Complaints form.

Our secure online form is coming soon. In the meantime, phone 0494 722 663 or email complaints@findingpath.com.au.

Making a complaint will not negatively affect the support you receive from Finding Path.

External complaints

You can also contact the NDIS Quality and Safeguards Commission about concerns relating to the quality or safety of NDIS supports or services.

You do not have to resolve the issue with Finding Path before contacting the Commission.

Immediate danger

If someone is in immediate danger or there is an emergency, call 000.
